Ruby Survey/Questionaire ?

Hi there,

Is there a gem or script available for a survey/Questionaire..so i can
take surveys from the visitors of our website...

I want to offer multiply-choice options...and each answer has a
score...based on the score the visitors is redirect to a specified page.
